How to get a library card and important information about your card
Applying for a Library Card
You can apply for a library card in person during regular open hours at the Circulation Desk, or at any Chemeketa Cooperative Regional Library Service (CCRLS) member library. Children under 16 will need to bring a parent or legal guardian with them to sign the application form, and provide picture ID (see below). The child's library card allows check out of all library materials (the Kid's C.A.R.E. Card is limited to children and young adult materials), including the Internet, unless a parent requests a no Internet access block.
Required Idenification
You will need to bring in a valid Picture ID (such as a current Oregon Driver's License or Oregon State ID) that shows your current residence. If you do not have the current residence on your Picture ID, we will also need a current utility bill for address verification.
Other forms of acceptable Identification are:
- US Passport AND a current utility bill
- US Green Card AND a current utility bill
- Military ID AND a current utility bill
Important Information about Your Library Card
- Items checked out on your card are your responsibility. Treat your card as you would a credit card. Lending your card to others is not a good idea.
- If you lose your card, please let us know immediately. We can place a block on the card to prevent others from using it. Replacement cards cost $1.00.
- Please inform us if you move or change your telephone number or email address. Our electronic system for notifying patrons about reserves and overdue fines requires current contact information.
- If you have not used your card within two years, you may be asked to apply for a new one.
